Intervention Specialist
Case Managers assist clients by providing a myriad of services in the home, the community and in the psychosocial day program. These services may include counseling, assistance with medication compliance and monitoring, budgeting or providing assistance in obtaining benefits. Intervention specialist provides services both in the offices and in the homes to assure clients' needs are being met.
Psychosocial Program (Community Integration)
Activity Therapists, in conjunction with clients, conduct a day program that is psychosocial in nature. The program operates five days a week, is geared toward assisting clients with activities of daily living to maintain their optimal levels of independent functioning and to prevent unnecessary hospitalizations. Opportunities are provided for participation in local and cultural events. A nutritional breakfast, lunch and snacks are served daily.
Outpatient & Medical Services
Supportive psychiatric evaluation, treatment and supervision
is provided by psychiatrists on contract with SGT and by nurses
and other staff members who have had training specific to working
with clients who have serious and persistent mental illnesses.
Medical problems of clients are closely monitored and referrals
are made to appropriate physicians of the client's choice, as
necessary. Co-occurring/substance disorders are assessed, and
appropriate treatment is provided or referred as needed or requested.
Crisis Intervention
On a weekly rotating basis, a staff member is 'on-call' and available by cell phone, pager or telephone 24 hours per day, 7 days per week. Staff receive additional pay to be 'on-call'.
Client Employment Services
A work training program that makes available 5-20 hours of
work per client per week is provided at SGT. Trained/certified
job coaches provide supervision and on the job training. Additionally,
clients may be placed in the community to work. Each client
is paid at least minimum wage with appropriate state and federal
taxes withheld from each payroll check. When the client and
case manager budget for savings, the amount is deposited directly
into the client's bank account.
Transportation
Transportation is provided for clients to and from physician
appointments, the Client Employment Service Program when employed
at SGT, and psychosocial day program outings.
Clients residing at Springwood Manor Apartments pay a fee for
transportation.
Financial & Accounting Services
In most cases, SGT is the representative payee for the client's
Social Security or SSI income. Individual accounts are maintained
for each client; bank statements are reconciled monthly and
checks are disbursed as needed upon reasonable written requests
from clients.
Food Services
Food services prepares and serves two meals and two snacks five days per week for
clients attending the psychosocial day program at Whittington Avenue and Spring Street. Additionally,
in Hot Springs, three meals and two snacks are prepared and served to clients residing
at Oakhill Lodge and Fairweather Manor.
